Job Summary

We are seeking a highly skilled Social Worker-Case Management to join our team at Ascension St. Vincent Hospital in Indianapolis, IN. As a key member of our interdisciplinary care team, you will be responsible for coordinating the overall plan of care for patients, from admission to discharge. This is a full-time position, working Monday through Friday, 8am-4:30pm.

Responsibilities
  • Coordinate the overall interdisciplinary plan of care for patients, integrating patient/family preferences and values.
  • Assess, evaluate, recommend, and treat legal, neglect, substance abuse, crisis, psychosocial, and adjustment needs.
  • Advocate for resources and removal of barriers.
  • Maintain ongoing dialogue with supervisor and care transition team members to ensure effective implementation and reevaluation of health plans.
  • Act as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care.
Requirements
  • Bachelor's degree in Social Work required.
  • 3 years of experience preferred.
  • Clinical Counselor, Counselor, or Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board required.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) or American Case Management Association (ACMA) preferred.
