Key Responsibilities- Collaborate with the patient care team to identify and address barriers to care and discharge
- Develop and maintain accurate and comprehensive documentation, including the Individual Plan of Care (IPoC)
- Communicate effectively with patients, families, and caregivers to ensure informed decision-making and optimal outcomes
- Coordinate weekly patient care team conferences to refine treatment plans and achieve identified patient goals
- Review patient assignments and identify potential missed comorbid conditions, communicating findings to the HIM team
- Current licensure as a Registered Nurse, Social Worker, or Healthcare professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist)
- Certification in Case Management or Rehabilitation Nursing preferred
- Minimum of 2 years experience in an inpatient setting, with acute/rehabilitation hospital experience preferred
- Effective oral and written communication skills in English, with additional languages a plus
- Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
