We are seeking a highly motivated and experienced professional to join our team as a Case Manager at Renewal Inc. The successful candidate will be responsible for providing guidance, direction, and management of reentrants and their activities.
Key Responsibilities- Act as the primary contact for reentrants and assist them with their concerns and issues.
- Determine reentrants' social service needs and assist in creating and implementing Individual Renewal Plans.
- Enforce compliance with program guidelines and assist the Case Management Supervisor in the functioning of the center's daily activities.
- Make appropriate referrals to Renewal, Inc. programs as well as outside agencies based on the reentrant's established needs.
- Assist in initiating and developing policies and procedures as well as maintaining reentrant records.
- Assist reentrants in establishing goals for their aftercare plan and release from the program into the community.
- Bachelor's degree in Criminal Justice, Psychology, Counseling, or Social Work.
- Minimum of two years of experience in a related field.
- Master's degree preferred.
- Knowledge of Cognitive Behavioral Therapy and Motivational Interviewing practices.
- Excellent communication and problem-solving skills.
- Ability to work effectively with all levels of management, employees, and reentrants.
