Job Description: We are seeking a Medical Care Coordinator to join our team at HealthCare Support. As a Medical Care Coordinator, you will be responsible for coordinating patient care services and evaluating patients' health status through in-person or telehealth interviews.

**Key Responsibilities:**
  • Coordinate patient care services to ensure timely and effective delivery of care.
  • Evaluate patients' physical, mental, and social conditions through in-person or telehealth interviews.
  • Document assessment findings accurately in the UAS-NY system in compliance with state regulations and organizational policies.
**Requirements:**
  • Bachelor's degree in Nursing or a related field.
  • Active and unrestricted New York State RN license.
  • Minimum 2 years of experience performing UAS assessments.
  • 3+ years of experience in managed care programs.
  • 5+ years of experience in care management, utilization review, quality assurance, or discharge planning.
