The Home Care Field RN plays a vital role in delivering high-quality care to patients in their homes. This position requires a registered nurse with a strong background in community health and home care nursing.
Key Responsibilities- Conduct thorough assessments of patients and families to determine home care needs.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Provide hands-on nursing care, including administering medications and treatments as prescribed by physicians.
- Counsel patients and families on health care and wellness.
- Coordinate with community health professionals to ensure seamless care delivery.
- Participate in on-call duties and be available to address patient needs outside of regular working hours.
- Current RN licensure in the state.
- Minimum of two years of experience in home care or public health nursing.
- Excellent observation, problem-solving, and communication skills.
- Ability to work independently with minimal supervision.
Salary Range: $100K - $120K
