Core Responsibilities
  • Attend to personal care needs based on patient assignments and respond promptly to requests.
  • Communicate effectively with patients and families, ensuring ethical and respectful care while maintaining confidentiality.
  • Ensure patient safety by using equipment properly (e.g., foot stools, side rails, O2).
  • Provide personal care services, including bathing, oral hygiene, and changing bed linens as required.
  • Assist patients with dressing and undressing as needed.
  • Plan and prepare nutritious meals, including shopping when necessary.
  • Support patients during feeding as assigned.
  • Monitor and record vital signs such as temperature, pulse, and blood pressure as per care plans.
  • Observe and care for patients' skin to prevent tissue breakdown.
  • Report any significant changes in patient condition to the Case Manager.
  • Assist with ambulation and exercises as directed by healthcare professionals.
  • Facilitate normal range of motion and positioning as instructed.
  • Support self-administered medications as permitted by state regulations.
  • Provide assistance with light laundry tasks as assigned.
  • Perform range of motion exercises and other simple procedures as ordered.
  • Offer respite for patients and their families when on-site, as appropriate.
  • Maintain cleanliness and orderliness in the patient's living area as assigned.
  • Adhere to documentation and care standards, maintaining professional conduct.
  • Participate in quality assessment and performance improvement initiatives.
  • Respect the personal needs and environment of patients and families.
Qualifications
  • Must meet state training requirements.
  • Completion of at least seventy-five hours of classroom and supervised practical training, with specific hours allocated for each.
  • A minimum of six months of experience in providing personal care.
  • Must be at least eighteen years old.
  • Ability to read, follow written instructions, and document care provided.
  • Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
  • Positive references from previous employers.
  • Ability to work independently with minimal supervision.
  • Comfortable with issues surrounding death and dying.
  • Maintain a calm demeanor in patients' homes.
  • Current CPR certification is required.
  • Exhibit tact, patience, and good personal hygiene.
  • Successful completion of a probationary period to ensure competency in required qualifications.
  • Strong oral and written communication skills.
  • Good organizational abilities.
