Key Responsibilities
- Deliver personal care services as assigned, ensuring prompt attention to patient requests.
- Communicate effectively with patients and families, maintaining confidentiality and respect.
- Ensure patient safety by using equipment properly and adhering to safety protocols.
- Provide personal care, including bathing, grooming, and changing bed linens as needed.
- Assist patients with dressing and undressing as required.
- Plan and prepare nutritious meals, including shopping for necessary items.
- Support patients during meals, assisting with feeding when necessary.
- Monitor and document vital signs, including temperature, pulse, and blood pressure, as per care plans.
- Observe and report any significant changes in the patient's condition to the Case Manager.
- Assist with mobility and exercises as directed by healthcare professionals.
- Provide basic assistance with medications as permitted by state regulations.
- Maintain cleanliness and organization in the patient's living space.
- Participate in quality improvement initiatives and adhere to documentation standards.
- Respect the personal needs and environment of patients and their families.
Qualifications Required
- Completion of state-required training programs.
- A minimum of seventy-five hours of classroom and supervised practical training.
- At least six months of experience in providing personal care.
- Must be at least eighteen years old.
- Ability to read, follow instructions, and document care provided.
- Understanding of hospice care principles and comfort in providing care to terminally ill patients.
- Strong references from previous employers.
- Ability to work independently with minimal supervision.
- Comfortable dealing with issues surrounding death and dying.
- Exhibit patience, tact, and maintain personal hygiene standards.
- Possess current CPR certification.
- Demonstrate strong communication and organizational skills.
