Key Responsibilities

  • Deliver personal care services as assigned, ensuring prompt attention to patient requests.
  • Communicate effectively with patients and families, maintaining confidentiality and respect.
  • Ensure patient safety by using equipment properly and adhering to safety protocols.
  • Provide personal care, including bathing, grooming, and changing bed linens as needed.
  • Assist patients with dressing and undressing as required.
  • Plan and prepare nutritious meals, including shopping for necessary items.
  • Support patients during meals, assisting with feeding when necessary.
  • Monitor and document vital signs, including temperature, pulse, and blood pressure, as per care plans.
  • Observe and report any significant changes in the patient's condition to the Case Manager.
  • Assist with mobility and exercises as directed by healthcare professionals.
  • Provide basic assistance with medications as permitted by state regulations.
  • Maintain cleanliness and organization in the patient's living space.
  • Participate in quality improvement initiatives and adhere to documentation standards.
  • Respect the personal needs and environment of patients and their families.


Qualifications Required

  • Completion of state-required training programs.
  • A minimum of seventy-five hours of classroom and supervised practical training.
  • At least six months of experience in providing personal care.
  • Must be at least eighteen years old.
  • Ability to read, follow instructions, and document care provided.
  • Understanding of hospice care principles and comfort in providing care to terminally ill patients.
  • Strong references from previous employers.
  • Ability to work independently with minimal supervision.
  • Comfortable dealing with issues surrounding death and dying.
  • Exhibit patience, tact, and maintain personal hygiene standards.
  • Possess current CPR certification.
  • Demonstrate strong communication and organizational skills.
