- Deliver personal care services based on patient assignments and respond promptly to their needs.
- Communicate effectively with patients and their families while maintaining confidentiality and respect.
- Ensure patient safety by using equipment correctly and adhering to safety protocols.
- Provide personal hygiene assistance, including bathing, grooming, and changing bed linens as required.
- Assist patients with dressing and undressing as needed.
- Plan and prepare nutritious meals, including grocery shopping when necessary.
- Support patients during meals and assist with feeding as assigned.
- Monitor and document vital signs as per care plans and demonstrate competency in these skills.
- Observe and care for patients' skin to prevent tissue breakdown.
- Report any significant changes in patient conditions to the Case Manager promptly.
- Assist with mobility and exercise as directed by healthcare professionals.
- Provide assistance with self-administered medications as permitted by state regulations.
- Support light housekeeping tasks related to patient care.
- Conduct range of motion exercises and other basic procedures as part of therapy services.
- Offer respite care for patients and their families when on-site.
- Maintain a clean and organized living environment for patients.
- Follow organizational documentation standards and uphold professional conduct.
- Engage in quality improvement initiatives and team activities.
- Respect the personal needs and environment of patients and their families.
- Completion of state-required training.
- A minimum of seventy-five hours of classroom and supervised practical training, including sixteen hours of classroom training prior to practical experience.
- At least six months of experience in providing personal care.
- Must be at least eighteen years old.
- Ability to read, comprehend, and document care instructions.
- Understanding of hospice care principles and comfort in providing care to terminally ill patients.
- Positive references from previous employers.
- Ability to work independently with minimal supervision.
- Comfortable with issues surrounding death and dying.
- Exhibit a calm demeanor in patient homes.
- Current CPR certification is required.
- Demonstrate patience, tact, and good personal hygiene.
- Successful completion of a probationary period to ensure competency in the role.
- Strong communication and organizational skills.
