Job Summary:

Liberty Health is seeking an experienced Home Care and Hospice Liaison Specialist to join our team. As a key member of our healthcare team, you will play a vital role in coordinating patient services and facilitating the transfer of patients to home health and hospice services.

Key Responsibilities:

  • Coordinate healthcare institution and physician office activities to ensure seamless patient care.
  • Establish and maintain relationships with healthcare institutions to facilitate patient transfers.
  • Coordinate patient services activities, including health education and community outreach.
  • Collaborate with healthcare professionals to ensure continuity of care.
  • Assist with recruitment and retention of qualified healthcare staff.
  • Stay up-to-date on Medicare/Medicaid home health and hospice regulations.

Requirements:

  • Bachelor's degree or equivalent in a healthcare-related field.
  • Minimum 2 years of experience in home health, hospice, or healthcare administration.
  • Strong knowledge of healthcare regulations and policies.
  • Excellent communication and interpersonal skills.
  • Ability to travel as needed.
