Program Overview:
The Renaissance Community Program is designed to support individuals in Montgomery County, offering 24-hour supervision in a community environment with a focus on behavioral health services. Participants reside in either communal homes or apartments tailored to their needs for structure and oversight. They engage in various activities, group sessions, day treatment programs, and receive assistance with medication management to foster their personal goals in living, learning, working, and socializing.
Role Summary:
The Case Manager is responsible for identifying resources necessary to address the unique needs of each participant. This role involves ensuring the execution of the service treatment plan and advocating for participants to access essential services, while also identifying program needs and service gaps.
Key Responsibilities:
Collaborative Planning
- Work as part of a multidisciplinary team to deliver services from a holistic and values-driven perspective.
- Assist in creating and reviewing recovery plans in partnership with program participants, clinical staff, and other stakeholders.
- Establish and maintain relationships with community organizations to facilitate access to available resources for program participants.
Direct Service Delivery
- Provide community-based support to program participants, focusing on education, resource connection, community engagement, and achievement of recovery goals.
Administrative Duties
- Complete all required assessments and documentation as mandated by RHD, funding sources, and licensing/accreditation bodies.
- Perform additional duties as assigned.
Additional Responsibilities:
- Evaluate and respond to the needs and service eligibility of individuals, both in-office and within the community.
- Facilitate prompt referrals to appropriate treatment and support services.
- Assist with resource acquisition and coordination, including Medicaid, Social Security, housing, and employment services.
- Provide information and referrals for behavioral health and other human services, collaborating with relevant agencies as necessary.
Qualifications:
- Bachelor's Degree in Human Services or a related field.
- Intermediate computer proficiency.
- Preferred: One year of Case Management experience.
- Required: Two years of general work experience supporting individuals in behavioral health, addiction recovery, or IDD services.
Working Conditions:

- Community-based work within the program's service delivery area.
Physical Requirements:
- Medium lifting: up to 50 pounds occasionally, and/or up to 20 pounds frequently, and/or up to 10 pounds constantly.
Essential Physical Activities:
- Standing or sitting for extended periods.
- Walking and using hands for various tasks.
- Climbing stairs or ladders.
- Effective communication and visual assessment.
