Job Title: Social Worker

At Polara Health, we are seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing direct service to clients and their families, assessing and counseling therapy services, and seeking resources to address unmet needs.

Key Responsibilities:
  • Collaborate with the interdisciplinary treatment team to develop and implement individualized treatment plans.
  • Conduct thorough social work assessments for patients, including psychosocial, ASAM, and PRAPARE evaluations.
  • Participate in clinical staffing to provide relevant social work information to the treatment team.
  • Actively coordinate care for patients, identifying and addressing gaps in service needs, including social determinants of health.
  • Facilitate individual and group therapy services for patients.
  • Prepare applications and referral packets for next-level care services.
  • Demonstrate effective engagement skills, inspiring hope and promoting recovery and resiliency.
  • Ensure all interactions are documented in the electronic medical record in accordance with policies and procedures.
Requirements:
  • Master's degree in Social Work, Counseling, Marriage and Family Therapy, or Addiction Counseling.
  • Two years of previous social services experience.
  • Strong ability to maintain confidentiality and handle highly confidential information with professionalism.
  • Ability to work independently and collaboratively within a team setting.
  • Excellent interpersonal communication, problem-solving, and team-building skills.
  • Detail-oriented with excellent organizational skills and flexibility.
  • Ability to work well under pressure in a fast-paced, time-sensitive environment with shifting priorities and multiple deadlines.
