Job Title: Social Worker

Polara Health is seeking a skilled Social Worker to join our team. As a Social Worker, you will provide direct service to clients and their families, assessing and counseling therapy services, and seeking resources to address unmet needs.

Key Responsibilities:
  • Collaborate with the interdisciplinary treatment team to develop and implement individualized treatment plans.
  • Conduct thorough social work assessments for patients, including psychosocial, ASAM, PRAPARE, and safety plans.
  • Participate in clinical staffing, providing relevant social work information to the treatment team.
  • Coordinate care for patients, ensuring services are coordinated with facility staff and outpatient teams/providers.
  • Identify and address gaps in service needs, including social determinants of health, and make recommendations to meet patient needs.
  • Facilitate individual and group therapy services for patients.
  • Prepare applications and referral packets for next-level care services.
  • Demonstrate effective engagement skills, inspiring hope and promoting recovery and resiliency.
  • Ensure all interactions are documented in the electronic medical record, following policies, procedures, and industry standards.
Requirements:
  • Master's degree in Social Work, Counseling, Marriage and Family Therapy, or Addiction Counseling.
  • Two years of previous social services experience.
  • Preferred qualifications: 4 years of clinical experience, independent or associate licensure, and knowledge of AZ Title-36 legal process.
  • Strong ability to maintain confidentiality and handle sensitive information.
  • Ability to work independently and collaboratively within a team setting.
  • Excellent interpersonal communication, problem-solving, and team-building skills.
