Job Title: Hospice Registered Nurse

St. Luke's Hospital is seeking a compassionate and skilled Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.

Key Responsibilities:
  • Provide comprehensive nursing care to patients and families, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Supervise and educate licensed practical nurses (LPNs) and home health aides (HHAs) to ensure high-quality patient care.
  • Document patient care and progress in a timely and accurate manner, adhering to agency policies and procedures.
  • Participate in bereavement programs and provide support to families and caregivers.
Requirements:
  • Current professional nurse licensure to practice in Pennsylvania.
  • Two years of experience as a professional nurse in nursing practice or previous community health and/or Hospice care as a professional nurse.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong problem-solving and critical thinking skills, with the ability to make sound judgments and decisions.
