About the Job

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support in the comfort of their own homes.

Job Summary

The successful candidate will be responsible for providing professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations. This will include assessing patient needs, developing and implementing individualized care plans, and providing skilled nursing interventions to promote optimal health outcomes.

Key Responsibilities
  • Provide professional nursing services to patients in their homes, including assessments, care planning, and interventions.
  • Collaborate with the RN Supervisor to develop and implement individualized care plans that meet the unique needs of each patient.
  • Utilize a holistic approach to care, addressing the physical, emotional, and social needs of patients and their families.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and transitions.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
