About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team of healthcare professionals. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities
  • Provide professional nursing services under the direction of a RN in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Assess patients' physical and emotional needs, developing and implementing individualized care plans to promote optimal health and well-being.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Document patient progress, identifying areas for improvement and implementing changes as needed.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
