About the Role:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities:

  • Deliver skilled nursing interventions to patients with various health conditions, focusing on rehabilitation, prevention, and overall well-being.
  • Collaborate with the healthcare team to develop and implement individualized care plans, ensuring seamless transitions and optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare professionals to ensure comprehensive care and address any concerns or questions.
  • Conduct thorough assessments, develop and implement care plans, and evaluate patient progress to ensure the highest level of care.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.

Requirements:

  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
