Hospice Case Manager

We are seeking a compassionate and skilled Hospice Case Manager to join our team at Falcon Healthcare. As a Hospice Case Manager, you will play a vital role in ensuring that our patients receive high-quality care and support in the comfort of their own homes.

Key Responsibilities:

  • Assess patient needs and develop personalized care plans to meet their physical, emotional, and spiritual needs.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families to promote independence and self-care.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Document patient information and care plans in a timely and accurate manner.

Requirements:

  • Current RN License – active and in good standing
  • Minimum of 1-year nursing experience preferred
  • Hospice experience is a plus
  • Current CPR certification
  • Strong organizational and communication skills
  • Proof of current auto insurance
  • Proof of Covid vaccine, medical exemption or religious exemption
