Position Overview:
This role involves overseeing clinical nutrition records and ensuring compliance with relevant regulations. The successful candidate will be responsible for maintaining accurate documentation of individuals' nutritional statuses and conducting thorough assessments.

Key Responsibilities:
  • Manage and coordinate medical records related to clinical nutrition.
  • Conduct annual assessments utilizing the Nutrition Care Process.
  • Participate in Individualized Service Plan (ISP) meetings.
  • Perform nutritional assessments, develop care plans, and document progress notes.
  • Advise the treatment team on strategies to enhance nutritional health.
  • Suggest modifications to meal plans based on nutritional evaluations.
  • Facilitate diet changes and provide necessary training to individuals and staff.
  • Observe individuals during mealtime across various service locations.
  • Deliver nutrition education to individuals, families, and healthcare providers.
  • Collaborate with an interdisciplinary team to assess and address individual nutritional needs.
  • Organize and conduct nutrition training sessions for staff, maintaining attendance records.
  • Implement corrective action plans as needed.
  • Ensure adherence to regulatory standards for the health and safety of individuals.
  • Utilize electronic health record systems proficiently.
  • May assist with additional assignments as required.

Qualifications:

Applicants must possess current registration as a Registered Dietitian (RD) or certification from the New York State Education Department in dietetics-nutrition, along with a minimum of 400 hours of supervised practice under a registered dietitian.
