The Salvation Army is seeking a skilled Case Management Specialist to provide support and guidance to clients enrolled in our life management program. As a key member of our team, you will develop tailored program plans, monitor progress through mentoring sessions, and act as an advocate to ensure clients have access to necessary services.
Key Responsibilities
- Develop and implement individualized program plans to meet client needs
- Provide guidance and support to clients through regular mentoring sessions
- Act as an advocate to ensure clients have access to necessary services and resources
- Collaborate with community agencies and organizations to provide comprehensive support
- Monitor client progress and adjust program plans as needed
Requirements
- Bachelor's degree in Social Work, Behavioral Science, or related field
- Three years of direct case management experience
- Strong communication and motivation skills
- Proficiency in Client Data Management Systems and outcome measurement
- Ability to create and assess program plans based on client needs
- Skilled at evaluating service levels and client progress
- Capable of handling diverse client interactions
- Experience in client interviews and data collection
- Good organizational skills with attention to detail
Working Conditions
- Meeting attendance requirements
- Proficient in English language communication
- Clear and effective oral and written communication
- Ability to manage client caseload with clarity and efficiency
- Capacity to handle multiple cases simultaneously
- Physical tasks include light lifting and walking
- Majority of work is seated with occasional standing or walking
- Work environment includes exposure to health risks and driving for off-site visits
