Job Summary
We are seeking a skilled Case Manager to join our team at The Salvation Army. As a Case Manager, you will be responsible for providing comprehensive, long-term case management services to clients participating in our life management program.
Key Responsibilities
- Interview and assess clients to determine their needs and develop individualized program plans.
- Provide mentoring and counseling sessions to clients to help them achieve their goals.
- Advocate for clients to acquire services that will enable them to cope with their environment functionally.
- Ensure compliance with funding requirements and evaluate the effectiveness of our services.
Requirements
- Bachelor's degree in Social Work, Behavioral Science, or a related field.
- Three years of experience providing direct case management social services.
- Knowledge of social service resources and agencies in the community.
- Effective communication and motivation skills.
Working Conditions
The Case Manager will work in a shelter environment and may be required to drive a vehicle to off-site meetings and home visits. The position requires the ability to work with clients of all types and to maintain accurate and complete case notes and client records.
