Job Summary
We are seeking a skilled Case Manager to join our team at The Salvation Army. As a Case Manager, you will be responsible for providing comprehensive, long-term case management services to clients participating in our life management program.
Key Responsibilities:
- Interview and assess clients to determine their needs and develop individualized program plans.
- Provide ongoing support and guidance to clients to help them achieve their goals.
- Collaborate with community agencies and resources to ensure clients receive the services they need.
- Monitor and evaluate client progress, making adjustments to program plans as needed.
- Ensure compliance with funding requirements and maintain accurate records.
Requirements
To be successful in this role, you will need:
- A Bachelor's degree in Social Work, Behavioral Science, or a related field.
- Three years of experience in case management or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
Working Conditions
This role requires the ability to work in a fast-paced environment and to maintain confidentiality and professionalism at all times. You will also need to be able to work effectively with clients from diverse backgrounds and to adapt to changing circumstances.
