About the Role

We are seeking a highly skilled Diabetes Educator and Care Coordinator to join our team at NYC Health + Hospitals. As a key member of our nursing staff, you will be responsible for developing and implementing educational programs and activities related to diabetes care and management.

Job Description

Your primary responsibility will be to coordinate the delivery of diabetes education and care services to our patients, ensuring that they receive the highest quality care and support.

You will work closely with our clinical team to assess the educational needs of our patients and develop tailored plans to meet those needs. Your expertise in diabetes education will be essential in providing guidance and support to our patients, helping them to manage their condition effectively and achieve optimal health outcomes.

The ideal candidate will have a strong background in nursing and diabetes education, with excellent communication and interpersonal skills. You will be able to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.

Key Responsibilities:

  • Develop and implement educational programs and activities related to diabetes care and management
  • Assess the educational needs of patients and develop tailored plans to meet those needs
  • Provide guidance and support to patients to help them manage their condition effectively
  • Work closely with the clinical team to ensure seamless delivery of diabetes education and care services
  • Maintain accurate records and reports on patient progress and outcomes

Requirements:

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA)

Salary: The estimated salary for this role is between $85,000 - $110,000 per year, depending on experience and qualifications.
