LPN Care Coordinator

We are seeking a Licensed Practical Nurse to coordinate shelter residents' ongoing treatment in the community at our adults family shelter in Manhattan.
The ideal candidate will have flexibility to work a schedule that includes evenings and weekends, taking vital signs, assisting with diabetes medication, and coordinating with Case Managers and Director of Social Services to ensure proper care and treatment plans are followed.
Key Responsibilities:
- Take vital signs, such as blood pressure, temperature, and weight
- Assist with diabetes medication and treatment
- Take medical histories and enter information in computer systems
- Coordinate with Case Managers, Senior Case Managers, and Director of Social Services to ensure proper care and nutrition and treatment plans are followed
- Follow up with community health care providers to ensure medical treatment plans are documented, monitored, and followed
Requirements
To be successful in this role, you should have a completion of an accredited program and licensure in New York as an LPN. Preferred qualifications include the ability to work independently and as part of a team, assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences, and bilingual skills.
