Patient Services Coordinator LPN Home Health Full Time

About the Role:
The Patient Services Coordinator LPN Home Health Full Time is a vital member of our team, responsible for ensuring seamless care coordination and continuity for our patients. This role requires strong communication and organizational skills, with the ability to manage multiple assignments and prioritize tasks effectively.
Key Responsibilities:
- Manage patient schedules, ensuring timely reassignment of visits and updates to agent unavailability.
- Initiate infection control forms as needed and send completed reports to HRD for upload.
- Provide backup support during busy periods, including receiving calls from field staff and assisting with weekly case conferences.
- Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to relevant staff.
- Complete requested schedules for add-ons and applicable orders, ensuring supervisory visits are scheduled.
- Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
- Prepare weekly agent schedules, reviewing for productivity and geographic issues before forwarding to the Branch Director for approval.
- Verify visit paper notes in the scheduling console as needed.
- Assist with internal patient transfers between branch offices.
- Perform lab report assessments, fax copies to doctors, and route to Medical Records Department as necessary.
- May be required to perform patient visits and participate in on-call rotation.
Requirements:
- Be a Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
- Have at least 1 year of home health experience.
- Prior packet review/QI experience preferred.
- Coding certification is preferred.
- Must possess a valid state driver's license and automobile liability insurance.
- Must be currently licensed in the State of employment if applicable.
- Must possess excellent communication skills, the ability to interact well with diverse individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
