Job Summary

We are seeking a highly skilled and compassionate ED Navigator LPN to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in ensuring seamless patient transitions and providing exceptional care to our patients and their families.

Key Responsibilities
  • Conduct thorough initial evaluations and engage with patients and their families to understand their unique needs and circumstances.
  • Identify and connect patients with appropriate resources and services to support their healthcare journey.
  • Utilize evidence-based behavior change strategies to educate patients and families on healthy habits and self-care practices.
  • Collaborate with healthcare providers and other stakeholders to ensure comprehensive and coordinated care.
  • Develop and implement personalized care plans that address the physical, emotional, and social needs of patients.
Requirements
  • Associate's degree in a related field or equivalent experience.
  • 1 year of experience in a clinical or social services setting, with a focus on patient care and transition coordination.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Proficiency in computer software and data entry systems.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
Preferred Qualifications
  • Bachelor's degree in a related field.
  • Experience in case management, community-based organizations, or healthcare settings.
  • Strong leadership and problem-solving skills, with the ability to think critically and make sound decisions.
