Job Summary

We are seeking a skilled and compassionate Community Health Nurse to join our team at South Shore Health. As a Community Health Nurse, you will provide high-quality nursing care to patients in their homes, promoting health, preventing illness, and improving quality of life.

Key Responsibilities
  • Conduct thorough assessments of patients' physical and emotional needs, developing individualized care plans to address their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education on self-care and disease management.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Document patient progress, identifying areas for improvement and implementing changes as needed.
  • Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
Requirements
  • Graduate of an accredited School of Nursing.
  • Bachelor of Science degree in nursing from a college/university accredited by the NLN is preferred.
  • Minimum one to two years of medical/surgical nursing experience is preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
