Key Responsibilities
  • Admit new patients to our skilled nursing program and schedule appointments with primary nurses.
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
  • Provide ongoing care and support to patients, including medication management, wound care, and education on self-care.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Graduate of an accredited School of Nursing.
  • Bachelor of Science degree in nursing from a college/university accredited by the NLN is preferred.
  • Minimum one to two years of medical/surgical nursing experience is preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Work Schedule

We are seeking a candidate who is available to work a flexible schedule, including 4 nine-hour days, 1 holiday per year, and 1 weekend requirement every 4th weekend. Day shift is preferred.
