About the Role

Vitability Health is seeking a highly skilled Psychiatric Nurse Practitioner to join our Behavioral Health team. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to our patients.

Responsibilities
  • Assessment and Diagnosis: Conduct comprehensive psychiatric evaluations to assess patients' mental health conditions, identify issues affecting their mental well-being, and develop individualized treatment plans.
  • Medication Management: Prescribe psychotropic medications and related treatments to manage symptoms of psychiatric disorders and improve patients' functional health status.
  • Therapeutic Interventions: Develop and implement evidence-based therapeutic plans to address mental health problems and psychiatric disorders.
  • Patient Education: Educate patients and their families about their mental health conditions, treatment options, and self-care strategies.
  • Collaboration and Communication: Work closely with psychiatrists, other healthcare professionals, and support staff to ensure seamless care coordination and effective communication.
  • Documentation and Record-Keeping: Maintain accurate and detailed patient records, adhering to confidentiality and regulatory requirements.
