Job Description

The Service Coordinator plays a vital role in supporting the success of Youth Pathways participants. As the primary point of contact, this individual is responsible for ensuring participants receive the necessary services and resources to achieve their goals.

Key Responsibilities
  • Collaborate with participants to develop personalized service plans that align with their employment and educational objectives.
  • Address any immediate barriers that may hinder participants' enrollment in appropriate services or activities.
  • Evaluate and advocate for the necessary services to support participants in achieving their goals.
  • Guide participants in making informed decisions that support their goal achievement.
  • Adapt case strategies based on participants' evolving needs and perspectives.
  • Participate in the development, implementation, and monitoring of service plans.
  • Schedule regular meetings with participants to review progress and provide support.
  • Coordinate internal activities and follow-up services post-program to ensure participants continue to receive necessary support.
  • Complete administrative tasks, including data entry and case note documentation.
Requirements
  • Bachelor's degree in human services, social work, education, or a related field.
  • At least 2 years of experience in case management or service delivery.
  • Experience working with economically disadvantaged youth, homeless individuals, justice-involved populations, and/or public assistance recipients.
  • Excellent communication and data entry skills.
  • Proficiency in MS Office, including Word and Excel.
