We are looking for a full-time Clinical Nurse Educator to coordinate healthcare and provide direct interventions as necessary for our adult residential programs. The Clinical Nurse Educator will also train and supervise Direct Support Professionals in medication management and the healthcare needs of the individuals we serve.

KEY RESPONSIBILITIES:
- Coordinate healthcare and wellness initiatives for individuals in compliance with all state and federal regulations.
- Act as a liaison and advocate for individuals' healthcare, wellbeing, and inclusion with all members of the medical and interdisciplinary teams.
- Assist individuals in accessing appropriate health services for their preventative, acute, and chronic health needs.
- Ensure that the health status and physical conditions of individuals are assessed, monitored, reported, and addressed promptly.
- Participate in and attend assigned medical appointments and relevant meetings as required.
- Respond to emergency medical situations in a timely and appropriate manner.
- Train unlicensed staff to meet the healthcare needs of individuals, including medication administration and monitoring.
- Implement and train staff on new and revised health-related policies and procedures as directed.
- Collaborate with Program Managers to oversee pharmacy and medication requirements.
- Order medical supplies as needed for your program.
- Maintain health-related records and complete documentation per company policy and applicable regulations.
- Provide insights on potential new admissions regarding healthcare needs.
- Develop and update individual-specific health risk plans to ensure appropriate medical care.
- Manage transitions of care to and from healthcare facilities.
- Provide nursing on-call services as required by the program.
- Act as a mandatory reporter for any suspected abuse, neglect, or exploitation.
QUALIFICATIONS:
- Current RN license in New Hampshire.
- Preferred experience in community or long-term nursing.
- Experience working with individuals with intellectual and developmental disabilities and/or mental health conditions.
- Valid Driver's License.
