About the Role

The RN MDS Case Manager at Providence Mount St. Vincent plays a vital role in delivering holistic care to our patients. As a skilled nurse, you will work closely with our interdisciplinary team to assess, plan, and implement individualized care plans that address the unique needs of each patient.

Key Responsibilities

  • Conduct comprehensive assessments of patients to identify their medical, emotional, and social needs
  • Develop and implement individualized care plans that address the patient's goals and priorities
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Provide education and support to patients and their families to promote self-care and independence
  • Participate in quality improvement initiatives to enhance the overall quality of care

Requirements

  • Current Washington State Registered Nurse License
  • Two years of experience in Long-Term Care, Transitional Care Unit, or equivalent experience
  • Bachelor's Degree in Nursing (preferred)
