About the Role

As a RN MDS Case Manager at Providence Mount St. Vincent, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients. This is a part-time position working day shifts.

Key Responsibilities
  • Provide holistic nursing care to patients in accordance with the PH&S vision, the PMSV Statement on Professional Nursing Practice, and the Washington State Registered Nurse scope of practice.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop strategies to address these needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance the delivery of care and promote a culture of excellence.
Requirements
  • Current Washington State Registered Nurse License.
  • Two years of experience in Long-Term Care (LTC), Transitional Care Unit (TCU), or equivalent experience.
