Responsibilities:- Develop, implement, and evaluate plans of care to address the physical, psychological, social, and spiritual needs of patients and their families.
- Coordinate home care services, ensuring continuity of care and identifying interdisciplinary care needs.
- Provide education, supervision, and counseling to patients and their families/caregivers on care needs and management.
- Accurately document patient/family services and maintain accurate records.
- Current licensure as a Registered Nurse in the state(s) of practice.
- Minimum of three years of nursing experience, preferably in critical care, oncology, long-term care, or medical/surgical backgrounds.
- Strong assessment skills, experience with symptom management, and excellent interpersonal skills.
- Ability to work as a member of an interdisciplinary team and communicate effectively with patients, families, and healthcare professionals.
