About the Role:- Perform comprehensive assessments and develop personalized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals, including discharge planners and social service workers, to ensure seamless care transitions.
- Conduct regular home visits to evaluate the quality of care and provide support to patients and their families.
- Participate in ongoing interdisciplinary assessments to ensure the best possible outcomes for patients.
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the State of MN.
- Minimum 2 years of current nursing experience, with a preference for case management experience.
- Ability to travel within a designated geographic area and possess reliable transportation.
- An annual salary range of $89,905 - $106,233, with a $6,000 hiring bonus.
- A comprehensive benefits package, including medical, dental, vision insurance, 401k, and HSA with company contribution.
