Role Summary

The Inpatient Registered Nurse CAP 2 serves as an entry-level position within the Clinical Advancement Program (CAP) for newly hired professionals who meet the CAP 2 criteria or have progressed from CAP 1. This role is responsible for delivering and coordinating clinically proficient care, which includes assessment, planning, intervention, and evaluation for a designated group of patients. Responsibilities may include delegating suitable tasks to direct patient care personnel, including RNs, LVN/LPNs, and Patient Care Technicians. As part of the kidney disease healthcare team, this role engages in decision-making, educational initiatives, leadership roles, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.

Key Responsibilities

All duties and responsibilities are expected to be executed in alignment with organizational policies, procedures, nursing standards, and applicable regulations.

- Execute essential functions under the supervision of the Supervisor and with guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse.

- Conduct ongoing, systematic collection and analysis of patient data before, during, and after treatment for assigned patients, documenting findings in the patient medical record, and making necessary adjustments to treatment plans while notifying relevant parties.

- Assess and document patient and family learning needs to provide initial and ongoing education.

- Identify and report variations in patient status to appropriate healthcare team members.

- Direct and deliver safe, effective patient care for assigned patients according to prescribed treatment procedures.

- Assess daily patient care needs and collaborate with direct and ancillary care staff as necessary.

- Initiate and coordinate communication with dialysis providers and relevant facility personnel to ensure continuity of care.

- Implement, monitor, and document patient responses to prescribed treatments, including notifying physicians of any adverse reactions.

- Administer medications as prescribed and document their justification and effectiveness.

- Participate in emergency response measures as needed.

- Serve as a resource and mentor for new and existing staff, exemplifying appropriate behaviors and attitudes.

- May provide specialized nursing care instructions to facility staff as needed.

- May assist in outpatient facilities on an as-needed basis.

- May act as a Preceptor for new employees.

Physical Demands and Work Environment

- The position involves direct patient care that may require heavy lifting and moving of patients.

- Frequent prolonged periods of standing are required, and the employee must be able to bend.

- The employee may occasionally need to move equipment weighing up to 200 lbs. and lift solutions weighing up to 30 lbs.

- The work environment is typical of a healthcare facility with controlled air temperature and moderate noise levels.

- Exposure to infectious materials may occur.

- Participation in on-call rotation, night, weekend, and holiday shifts may be required.

Supervision

Oversight of Patient Care Technicians/LPNs/LVNs/RNs may be assigned as a designated Nurse in Charge after meeting all training and experience requirements.

Education and Licensure

Current state licensure and CPR BLS Certification are required. Candidates must meet practice requirements in all states of employment.

Experience and Required Skills

Entry-level for RNs with a minimum of 2 years of Nephrology Nursing experience in the last 2 years as a Registered Nurse. Acute Dialysis experience is preferred.
