The Clinical Decision Unit (CDU) offers patients a short-term, comprehensive approach to clinical care.
Nursing Responsibilities:
Engages in the Nursing Process, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation of clinical care. This role emphasizes evidence-based practices, analytical thinking, and individualized patient care. The nurse coordinates care through effective partnerships, recognizing that compassionate care is essential for achieving optimal patient outcomes.
Patient-Centered Care:
Delivers relationship-based, patient-centered care that is tailored to the specific characteristics of the population served (e.g., age, gender, health conditions). This involves adapting service delivery to reflect an understanding of cultural diversity.
Always collaborates with patients and their significant others, utilizing appropriate methods for communication and engagement (e.g., bedside shift reports, interdisciplinary rounds, timely patient education) to ensure that patient goals are prioritized in the care plan.
Teamwork and Collaboration:
- Coordinates the delivery and documentation of safe, high-quality patient care, promoting a professional care delivery model. This includes diverse and inclusive communication methods, timely documentation, and fostering a respectful clinical environment.
Exhibits teamwork and collaboration by practicing in a fiscally responsible manner and supporting the recruitment and retention of healthcare team members.
Actively participates in performance improvement initiatives, engaging all stakeholders in research and evidence-based practices to enhance patient care quality. This includes leading activities that promote inter/intra-disciplinary collaboration and supporting shared governance at all levels.
Professional Development:
Completes all required professional competency assessments and ongoing education, including population-specific training.
Serves as a mentor and preceptor for other nursing professionals and students, ensuring a qualified workforce and fostering a healthy work environment.
Evidence-Based Practice:
Advocates for evidence-based practices, nursing research, and performance improvement initiatives. Participates in value analysis activities to evaluate patient care products and practices.
Adheres to all healthcare system policies, procedures, and clinical competencies, ensuring compliance with legal, regulatory, and accreditation standards, including safety goals.
Resource Management:
Proactively plans for patient care across the continuum while providing direct care.
Contributes to the development of protocols and procedures in collaboration with care managers and healthcare team members to achieve optimal practice outcomes, such as reducing readmission rates and adverse events.
Minimum Experience Required:
At least 2 years of current experience in the relevant practice area.
Experience with computer data entry; familiarity with Epic and 3M is preferred.
Minimum Education and Certification:
Graduation from an accredited nursing program is required.
Preferred:
Baccalaureate degree in nursing (BSN) from an accredited institution.
All certifications must be obtained upon hire unless otherwise specified.
Registered Nurse (Single State) or RN - Multi-state Compact certification.
Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications are required.
