About the Role
As a Field Care Advisor Nurse at Chapters Health System, you will play a vital part in providing compassionate care to patients and families in their homes, assisted living facilities, nursing homes, or hospitals. Your primary focus will be on delivering exceptional customer service while assessing patient needs, identifying immediate requirements, and implementing appropriate nursing interventions.

Responsibilities
* Make after-hours visits to patients, prioritizing their care and addressing concerns through effective communication and collaboration with healthcare professionals
* Assess patient/family needs, provide direct care, and manage physical and emotional comfort through timely interventions and follow-ups
* Collaborate with Interdisciplinary Groups (IDGs) to ensure quality and continuity of care, referring cases when necessary and consulting with other professionals as required
* Educate patients, families, caregivers, and healthcare professionals about disease processes, palliative interventions, caregiving, dying processes, and safety practices
* Report changes in patient conditions to IDG members or other health professionals, ensuring seamless communication and care coordination
* Attend patient deaths, prepare bodies for transport, oversee medication disposal, and provide bereavement support as needed

Requirements
* Current RN license in the state where employed, with a minimum of one year's experience in hospice or hospital settings preferred
* Previous EMR/EHR system experience and ability to effectively communicate in English (verbal and written), with bi-lingual Spanish proficiency an asset
* Mobile driver with valid driver's license and automobile insurance per Company policy, reliable transportation to meet patient demands
* Excellent critical decision-making, problem-solving, clinical assessment, and time management skills, with the ability to work in fast-paced environments and handle stressful situations professionally
