Responsibilities:
- Provide high-quality, patient-centered care to individuals in their homes, promoting independence and dignity.
- Conduct thorough assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care transitions.
- Collaborate with patients, families, and caregivers to educate and empower them to manage their health and well-being.
- Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction.
Qualifications:
- Graduate of an accredited school of practical nursing with a current license to practice in the state serviced.
- Current CPR or BLS Certification.
- Compliant with appropriate state certification/registration.
- Proof of eligibility to pass medications.
- One year hospital nursing experience.
