Job Summary

As a PRN Home Health LPN, you will provide quality, skilled nursing care to patients in the comfort of their own homes. Under the direction of a Registered Nurse, you will administer care and identify the physical, psychosocial, and environmental needs of patients.

Responsibilities
  • Administer quality, skilled nursing care to patients in the home
  • Verify Plan of Care prior to each visit and provide care according to physician orders
  • Perform ongoing assessment during each visit and document data in patient medical records
  • Communicate significant findings, problems, or changes in patient condition to supervising RN and/or Director of Professional Services and physician
  • Document nursing interventions, including patient response
  • Assess patient/caregiver's knowledge of disease process, treatment regimen, and/or medication administration
  • Provide basic and specialized teaching as ordered in the Plan of Care and based on patient needs
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure
  • Driver's license
  • Vocational school graduate
  • 1 year of experience in healthcare
