Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at HCA Virginia Healthcare at Home. As a PRN Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify patient needs and develop individualized care plans.
  • Provide education and support to patients and their caregivers on disease management, treatment regimens, and medication administration.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient progress and care provided in a timely and accurate manner.
Requirements
  • Current LPN licensure in the state of Virginia.
  • Basic Cardiac Life Support (BCLS) certification.
  • 1 year of experience as an LPN in a home health or similar setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
