Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a PRN Home Health LPN. As a member of our team, you will provide high-quality patient care, working in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services.

As a PRN Home Health LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families, providing personalized care and support in the comfort of their own homes.

Responsibilities
  • Administer quality, skilled nursing care to patients in the home, verifying the Plan of Care prior to each visit and providing care according to physician orders, assessment data, established standards and guidelines.
  • Perform ongoing assessments during each visit, documenting data in patient medical records and communicating significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician.
  • Assess patient/caregiver's knowledge of disease process, treatment regimen, and/or medication administration, providing basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs.
Requirements
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Licensed Practical / Vocational Nurse (LPN / LVN) licensure.
  • Drivers License.
  • Vocational School Graduate.
