Work Schedule: PRN, as required

Work Hours: Weekdays, with occasional weekend shifts.

Service Area: This role involves conducting patient visits in various home settings.

Your Role:

As a Speech Language Pathologist, you will assess homecare patients to identify speech or language disorders and implement appropriate therapeutic interventions. You will provide rehabilitative services for swallowing, speech, and language challenges, document your findings in clinical records, and communicate any changes in the patient's condition to the physician. Additionally, you will guide other healthcare team members and caregivers in supporting the patient’s speech improvement.

Your Contributions to the Team:

  • Conduct thorough evaluations to establish diagnoses and develop care plans in collaboration with physicians.
  • Monitor and document patient progress, reporting any significant changes in their condition.
  • Educate patients and caregivers on speech language improvement strategies, including adaptive equipment and home exercises.
  • Train other healthcare personnel, including home health aides, on relevant aspects of speech therapy.
  • Tailor and apply skilled interventions to optimize patient outcomes, regularly assessing their responses to treatment.

Qualifications for Success:

Minimum Requirements:

· Proficiency in the English language

· Moderate computer skills

· Master’s degree in Speech Language Pathology

· Valid Florida Speech/Language Pathology license

· Current Florida driver’s license

· Basic Life Support certification

Preferred Qualifications:

· Experience in home health settings

· Familiarity with regulatory standards

· Certification in Vital Stim therapy

· Minimum of 2 years of clinical experience as a speech language pathologist

· Experience with adult and geriatric rehabilitation

· Recent home health experience within the last 3 years

· Knowledge of homecare regulations and OASIS assessments
