Work Schedule: PRN, as required
Shift: Weekdays, with occasional weekend shifts.
Service Area: This role involves conducting patient visits in various locations.

Your Role:
As a Speech Language Pathologist, you will assess homecare patients to identify speech or language disorders and develop appropriate therapeutic interventions. You will provide rehabilitative services for swallowing, speech, and language disorders, document your findings in clinical records, and communicate any changes in the patient's condition to the physician. Additionally, you will guide other healthcare team members and caregivers in supporting the patient’s speech improvement.
Your Contributions to the Team:
- Conduct evaluations to establish diagnoses and create a plan of care in collaboration with the physician. Determine necessary interventions, duration, and frequency to meet anticipated goals.
- Monitor and document patient progress, reporting any reactions to treatment or changes in condition to the physician.
- Educate patients and caregivers on the speech language improvement program, including the use of adaptive equipment.
- Provide training to other healthcare personnel, including home health aides, on relevant aspects of speech therapy.
- Select and implement skilled interventions tailored to the patient’s needs, assessing their response and making necessary adjustments.
Qualifications for Success:
Minimum Requirements:
· Proficiency in the English language
· Moderate computer skills
· Master’s degree in Speech Language Pathology
· Valid Florida Speech/Language Pathology license
· Current Florida driver’s license
· Basic Life Support certification
Preferred Qualifications:
· Experience in home health settings
· Familiarity with regulatory standards
· Certification in Vital Stim therapy
· At least two years of clinical experience as a speech language pathologist
· Experience with adult and geriatric rehabilitation
· Recent home health experience within the last three years
· Knowledge of homecare regulations and OASIS assessments
