As a Speech Language Pathologist, you will:
- Deliver services in accordance with state regulations and the established care plan, collaborating with the healthcare team.
- Assess patients' functional abilities through diagnostic evaluations and assist in creating tailored therapy plans.
- Facilitate effective communication for patients, adapting methods to meet individual needs.
- Monitor and document patient progress, reporting any changes to the supervising nurse or physician.
- Educate patients, families, and caregivers on speech therapy techniques and their roles in the therapeutic process.
- Conduct patient visits as per the treatment plan, completing progress notes in line with organizational policies.
- Finalize patient evaluations and develop speech therapy plans adhering to state-specific guidelines, communicating outcomes promptly.
- Participate in team meetings and contribute to staff development initiatives as required.
- Provide training sessions for agency staff upon request.
- Submit written summaries of patient conditions to the supervising nurse and/or physician at regular intervals.
Qualifications:
- Current licensure in Speech Therapy in the state of practice.
- Valid CPR certification.
- Possession of a current driver's license and vehicle insurance, with access to reliable transportation.
