Position Overview:
Maxim Healthcare is seeking a dedicated Registered Nurse (RN) to join our Transition team.

This specialized unit consists of skilled nursing professionals focused on facilitating the safe and efficient return of patients to their homes while our primary nursing staff is being recruited.

The Transition Team is activated upon receiving a referral for home care services, ensuring that patients are promptly admitted to Maxim.

Transition Team nurses deliver care for all authorized hours in a home setting during a provisional period ranging from 1 to 90 days. During this timeframe, our office personnel will be actively recruiting to find full-time replacement home healthcare nurses.

New caregivers will have the opportunity to train and shadow alongside Transition Team nurses before they begin working independently with the patient.

Once the replacement nurses are adequately trained and scheduled, the Transition Team will transition to assist with another new admission.

Key Responsibilities:
  • Utilizes the nursing process to plan, implement, and evaluate patient/client care and services provided to clients receiving contracted services.
  • Initiates and adjusts the plan of care as directed, requesting necessary revisions from the prescriber as patient status and needs evolve.
  • Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals.
  • Responds to changes in patient/client condition and communicates these changes to the appropriate physician or supervisor.
  • Administers prescribed medications and treatments, monitoring and documenting the effectiveness of interventions.
  • Reviews the plan of care and program protocols, identifying areas for healthcare education, care coordination, goal setting, and discharge planning needs.
Qualifications:
  • Current Registered Nurse (RN) License for the state of practice.
  • Current Health Certificate as per facility RN contract or state regulations.
  • Current PPD or Chest X-Ray.
  • Current BLS certification.
  • Preferred: One year of prior Registered Nurse (RN) experience.
