About This Licensed Practical Nurse (LPN) Position

HopeHealth is seeking an experienced Licensed Practical Nurse (LPN) for its Rhode Island team.

This is a full-time, 40 hour per week position, with 3:00pm-11:30pm hours, 5 days a week. There is a rotating weekend commitment, located at the Hulitar Center in Providence.

This position is eligible for a bonus of up to $2,500.

What You'll Do
  • Conduct assessments, assist with admissions and discharge planning for IPU patients for home and nursing home patients.
  • Assess patient and family status based on the physiological, psychosocial, spiritual, and emotional needs presented.
  • Implement the plan of care under the direction of the nurse case manager and attending physician.
  • Demonstrate skill in interpersonal relationships with Hospice patient, families, co-workers and community support services.
  • Maintain accurate records in designated manner to record observations, problems and nursing actions which reflect the care given to Hospice patients
What We're Looking For
  • Current RI Nursing License
  • Two years hospice or oncology preferred
  • Must have well developed nursing assessment and clinical skills including solid documentation and medication dosage calculations
  • LPN required
  • Hospice/Palliative care nursing certification preferred
