LPN - Post Acute Care Specialist

Seattle Medical Post Acute Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a key member of our healthcare team, you will provide high-quality nursing care to our residents and work collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Key Responsibilities- Supervise and coordinate the delivery of nursing care to residents, ensuring that their physical, emotional, and social needs are met.
- Comply with all relevant state and federal regulations, as well as our organization's policies and procedures.
- Work collaboratively with our healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.
- Maintain the highest standards of professional nursing practice, including staying up-to-date with the latest evidence-based practices and technologies.
- Graduate of an accredited school of nursing.
- Current licensure in the state of employment.
- Demonstrated knowledge of the nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
