Nurse Practitioner

Citizens Medical Center is seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality patient care and collaborating with our multidisciplinary team to ensure the best possible outcomes for our patients.
Key Responsibilities- Provide direct patient care based on the plan of care and exercise judgment in delegating aspects of patient care to licensed and unlicensed staff members.
- Participate in professional development of self and staff to enhance patient-focused nursing practice.
- Perform timely and comprehensive assessments using appropriate techniques for data collection, analysis, interpretation, and documentation.
- Develop, implement, document, and evaluate the plan of care in collaboration with other team members.
- Actively participate in unit's performance improvement initiatives.
- Perform nursing interventions with emphasis on patient rights, informed consent, patient confidentiality, HIPAA compliance, adaptation of care according to age, cultural, and ethical considerations.
- Administer medication according to facility standards and demonstrate safe and accurate administration of medication/control substances.
- Observe and report patient progress and changes in patient status to the physician and document in the medical record.
- Screen for and communicate discharge needs on a regular basis in collaboration with the multidisciplinary team members.
- Accurately record vital signs, intake, and output and other physical observations on the appropriate forms.
- Focus on prevention/care of pressure ulcers.
- Evaluate the need for and consistently provide and document education provided to the patient and/or patient caregivers.
- Evaluate with the physician or independent allied health professional the need to initiate restraints, monitors use of restraints, and evaluates on a regular basis according to hospital policy the ability to discontinue restraints.
- Monitors appropriate pain management and performs interventions.
- Perform patient care activities; assisting with ADLs, mobilization of patients, drain, dressing, wound care, associated therapeutic devices, and nutritional needs.
- Obtain physician orders to meet the needs of the patient.
- Develop and maintain proficiency in the use of equipment and facilitates maintenance of equipment by completing maintenance requests.
- Demonstrate 'Service Excellence Initiatives' by presenting a positive first impression; anticipates customer needs, and is actively involved in the role of Team Player.
- Follows universal precautions.
- Complies with Employee Health requirements for employment.
- Works in a manner that promotes safety, adhering to dress code policy.
- Knows role in emergency situations and reports unsafe practices to management.
- If assigned as charge nurse: Uses knowledge of the staffing plan, skill mix/staff competencies, and patient acuity to develop an appropriate shift assignment.
- Able to assess the need for adjusting staff resources based on changes in census or acuity.
- Coordinates with other disciplines and department the process of admissions, transfers, and discharges.
- Uses effective leadership and organizational skills for problem-solving; mentoring and directing staff; and reporting issues on a timely basis to physicians, assistant directors of nursing, and/or other department leaders or their representatives.
- Graduate of accredited college, university, or school of nursing.
- Current license or permit to practice as a Registered Nurse in the State of Texas.
- CPR (BLS) certification.
- TNCC or ENPC certification every 3 years.
- ACLS and PALS recertification biannually.
