Job Description

At PATH, we're committed to empowering individuals and families to break the cycle of homelessness. As a Housing Navigator, you'll play a vital role in helping our clients achieve stability and independence.

Key Responsibilities:

  • Conduct comprehensive intake and needs assessments to identify clients' housing goals and develop individualized service plans
  • Provide referrals, linkages, and support to resources that help clients achieve their goals, including medical, mental health, and financial resources
  • Collaborate with clients to evaluate their strengths and challenges and develop strategies to address short-term and long-term goals
  • Maintain accurate and up-to-date client files and documentation, including case notes and program-specific assessments

Requirements:

  • 2+ years of experience in healthcare, case management, or working with vulnerable populations
  • Strong analytical and problem-solving skills, with the ability to think creatively and outside the box
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and colleagues
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
