Case Manager

Project 180 is seeking a skilled Maternal Health Case Manager to join our team. As a Case Manager, you will work with clients who have a forensic dual-diagnosed background, providing intensive, wraparound, field-based case management services to help them establish permanent supportive housing.
Key Responsibilities:
- Provide intensive case management services to clients with a forensic dual-diagnosed background.
- Develop and maintain knowledge of available community resources and link clients to these resources.
- Teach prosocial and recovery skills to clients to promote long-term stability in the community.
- Collaborate with external partners to ensure high-quality, integrated care.
- Maintain comprehensive case notes and submit billing records in accordance with SSG guidelines.
Requirements:
- Bachelor's degree in any field from an accredited college or university, and at least 1 year of case management experience.
- Experience working with culturally diverse populations.
- Valid CA driver's license, proof of insurance, and own vehicle required.
- Must not be on any type of legal supervision (probation or parole).
